We have defined a lingual orthodontic concept called « Essential Minimum » Lingual technique which means that has been kept only what is really essential to make the treatment successful.
All malocclusions even the most difficult can be treated with this technique that can be reproducted and used by all orthodontists who want to give a premium service to their adult patients.
Characteristics of the « Fillion » Lingual technique :
1- Laboratory procedure
We used an indirect laboratory procedure to position the lingual brackets. The bracket postioning is made directly on the malocclusion model with the TAD(Torque Angulation Device) and the Bracket Positionning Device (BPD).
The anterior brackets are bonded at the same distance from the labial surface. Then the brackets are transferred directly to the patient by a silicon tray. The compensation of difference of labio-lingual thickness allows to reduce the number of bends added to the wires and get a better finishing more easily. This procedure has been described at the American Lingual Orthodontic Association meeting in Palm Spring(1989) and called B.E.S.T system, Bonding wtith Equal Specific Thickness.
Each brackets is bonded on teeth according to 4 specific references defined by the pratician : Angulation, Torque, Bonding height and Thickness (distance from slot to the labial thickness).
These references are selected by the practician from a standard laboratory prescription or from a 3D virtual Set-Up with virtual bracket positionning (ORAPIX System).
The technician reproduces on the real plaster model the virtual bracket positionning : same bonding height, and same thickness.
This Cad/Cam bracket positionning allows to get a more precise tooth position and then a better and easier finishing.






2- Individualized Archiwe Template
We use an individualized template wich is able to give the precise form of lingual archwires for each patient.
This template makes us save a precious clinic time during archbending.
This template is provided by the 2D DALI software (Fillion-1988) or by the 3D-3Txer software (ORAPIX).



3- Indirect bonding procedure
This procedure includes microsandblasting, orthophosphoric acid etching and bonding with lightcuring material as : Orthosolo + Flowtain LV (Reliance orthodontic products) or Resin Reinforced Glass Ionomer Cement (GC-Fuji LC) or selfcuring material as Maximum Cure (Reliance Orthodontic Products).

4- Use of confortable appliance
We use the smallest lingual brackets, called STb, designed by Dr.Scuzzo and Dr. Takemoto (sold by ORMCO).
These brackets (1.5 mm thickness) improve greatly the comfort of orthodontic treatment for the patient, giving minimum discomfort in terms of perception of the appliance and creating any speech disturbance.
With these brackets, lingual orthodontics is no longer a unconfortable technique.


5- Low friction- Low force concept
Due to the specific design of STb brackets, the use of .010 or .012 NITI wire allow to get a good alignement and leveling in a shorter time.



6- Frictionless retraction mechanics
The « frictionless mechanics » (FM) used for extraction cases allow to retract the anterior segment with a lower level of force.
These light force set on lever arms avoid « bowing effect », and provides a « bodily » and faster movement.
The « frictionless mechanics » used with a full-size combination wire (rectangular in the anterior area .018 x .025 and round in the posterior area : .018) allows to get a good torque control during the retraction sequence.
The « frictionless mechanics » can divide by two the time of the retraction sequence.



7- Safe anchorage
The control of anchorage is assured by using mini-screws or micro-implants set in the palate .
In combination with frictionless mechanics, the mini-screws provide the most reliable anchorage to reach treatment goals in a faster and more predictable way.

8- Easier finishing
Due to the 4 references used for bracket positionning (B.E.S.T system), the accuracy of wire shaping and bonding, the STb brackets with a more precise slot, the advantages of the Frictionless mechanics and mini-screws for anchorage, the finishing sequence is no longer the most difficult sequence of the treatment.
